$25,000 24 Hour Accidental Death and Dismemberment - If within 1 year from the date of accident an injury results in a covered loss, we will pay the percentage of the principle sum listed in the table found in the synopsis of coverage. Family membership provides coverage for your spouse and children (50% for covered spouse, 60% if there are no dependant children, 20% for dependant children, 25% if there is no covered spouse).
Lifestyle Insurance Protector - If injury to you results in death within 90 days of the accident that caused the injury the company will pay an accidental death benefit maximum amount of $1,000. You can also receive a short-term permanent total disability benefit of $5,000/month if, as a result of an injury, you become permanently totally disabled within 90 days of the accident that caused the injury. No loss will be covered if it occurs more than 365 days after the date of the covered accident.
If, within 1-year from the date of an Accident covered by this Certificate, Injury from such Accident, results in Loss listed below, we will pay the percentage of the Principal Sum set opposite the loss in the table below. If the Covered Person sustains more than one such Loss as the result of one Accident, we will pay only one amount, the largest to which he is entitled. Loss of a hand or foot means complete Severance through or above the wrist or ankle joint.
Loss of sight means the total, permanent loss of sight of the eye. The loss of sight must be irrecoverable by natural, surgical or artificial means.
Loss of speech means total, permanent and irrecoverable loss of audible communication.
Loss of hearing means total and permanent loss of hearing in both ears which cannot be corrected by any means.
Loss of a thumb and index finger means complete Severance through or above the metacarpophalangeal joints (the joints between the fingers and the hand).
"Severance" means the complete separation and dismemberment of the part from the body.

If Injury to You results in death within 90 days of the accident that caused the Injury the Company will pay the Accidental Death Benefit Maximum Amount shown in the Benefit Schedule.
* Permanent Total Disability Benefit
- Short Term Benefit1 - $5,000 ($2,500 for Spouse)
- Long Term Benefit2 - $10,000 ($5,000 for Spouse)
[PERMANENTLY TOTALLY DISABLED/PERMANENT TOTAL DISABILITY means:
1. That the Insured Person suffered any of the following:
1. loss of both hands or feet; or
2. loss of one hand and one foot; or
3. loss of speech and hearing in both ears; or
4. Hemiplegia; or
5. Paraplegia; or
6. Quadriplegia;
2. The Insured Person is permanently unable to perform the material and substantial duties of any occupation for which the Insured Person is qualified by reason of education, experience or training;
3.The Insured Person requires the supervision of a Physician, unless the maximum point of recovery has been reached.
"Loss of a hand or foot" means complete severance through or above the wrist or ankle joint.
"Hemiplegia" means the complete and irreversible paralysis of the upper and lower Limbs of the same side of the body.
"Limb(s)" means entire arm or entire leg.
"Paraplegia" means the complete and irreversible paralysis of both lower Limbs.
"Quadriplegia" means the complete and irreversible paralysis of both upper and both lower Limbs.]
These insurance benefits are underwritten by National Union Fire Insurance Company of Pittsburgh, P.A. and are subject to the exclusions , limitations, terms and conditions of coverage as set forth in the description of coverage provided in your membership materials and insurance policy SRG9115152 issued to American Advantage Association. Benefits are not payable in connection with a Pre-Existing Condition during the initial 24 consecutive months the Insured Person has been enrolled for coverage, including any waiting period for coverage eligibility. All payable Maximum Amounts specified herein will be reduced 50% if You are age 70, or older, on the date of the accident for which benefits are payable.
1 - If, as a result of an Injury, You are rendered Permanently Totally Disabled within 90 days of the accident that caused the Injury, and if the Permanent Total Disability due to that Injury continues for the Short Term Waiting Period specified in the Benefit Schedule, the Company will pay a monthly benefit equal to the Permanent Total Disability Short Term Benefit Maximum shown in the Benefit Schedule starting with the 1st consecutive month of Short Term Permanent Total Disability following the Waiting Period.
2 - If as a result of an Injury, You are rendered Permanently Totally Disabled within 90 days of the accident that caused the Injury, and if the Permanent Total Disability due to that Injury continues for the Long Term Waiting Period specified in the Benefit Schedule, the Company will pay a monthly benefit equal to the Permanent Total Disability Long Term Benefit Maximum shown in the Benefit Schedule starting with the 1st consecutive month of Long Term Permanent Total Disability following the Waiting Period. Benefits will not be paid for a Covered Person's loss which:
(1) Is caused by or results from the Covered Person’s own:
a. Intentional self-inflicted Injury, suicide or any attempt thereat;
b. Voluntary
self-administration of any drug or chemical substance not prescribed
by, and taken according to the directions of a doctor (Accidental
ingestion of a poisonous substance is not excluded.);
c. Commission or attempt to commit a felony;
d. Participation in a riot or insurrection;
e. Driving under the influence of a controlled substance unless administered on the advice of a doctor; or
f. Driving
while Intoxicated. “Intoxicated” will have the meaning determined
by the laws in the jurisdiction of the geographical area where the loss
occurs;
(2) Is caused by or results from:
a. The
Covered Person being Intoxicated. (“Intoxicated” will have the meaning
determined by the laws in the jurisdiction of the geographical area
where the loss occurs); or
b. Any narcotic, drug, poison, gas
or fumes voluntarily taken, administered, absorbed or inhaled, unless
prescribed by a doctor;
c. Declared or undeclared war or act of war;
d. Injury
sustained while in the service of the armed forces of any
country. When the Covered Person enters the armed forces of any
country, we will refund the unearned pro rata premium upon request;
e. Aviation, except as specifically provided in this Policy;
f. An Injury that is caused by flight in:
i. An aircraft, except as a fare-paying passenger;
ii. A space craft or any craft designed for navigation above or beyond the earth's atmosphere; or
iii. An ultra light, hang-gliding, parachuting or bungi-cord jumping;
(3) Travel in or upon:
a. A snowmobile;
b. Any two or three wheeled motor vehicle;
c. Any off-road motorized vehicle not requiring licensing as a motor vehicle
(4) Sickness,
disease, bodily or mental infirmity or medical or surgical treatment
thereof, bacterial or viral infection, regardless of how
contracted. This does not include bacterial infection that is the
natural and foreseeable result of an accidental external bodily injury
or accidental food poisoning;
(5) Services or treatment rendered by a doctor, nurse or any other person who is:
a. Employed or retained by the Policyholder; or
b. Who is the Covered Person or a member of his immediate family;
(6) Elective treatment or surgery, health treatment, or examination where no Injury is involved;
(7) Cosmetic surgery, except for reconstructive surgery on an injured part of the body;
(8) That
part of medical expense payable by any automobile insurance policy
without regard to fault. (Does not apply in any state where prohibited);
(9) Any
loss which is covered by state or federal worker's compensation,
employers liability, occupational disease law, or similar laws;
(10) Hernia of any kind; or any bacterial infection that was not caused by an Accidental cut or wound.
